Use this form to ask for a correction to your T2202 or T4A tax slip (for example, changes to SIN, personal information, tuition amounts, months of study, or payment timing).

This form is for submitting review for corrections only; it does not provide tax advice.
Not all submissions will be eligible for corrections.

Step 3: Once your record is updated and shows correctly, submit this form so we can review and, if appropriate, issue an amended T2202 or T4A.
